On behalf of the entire staff, we would like to welcome you to Seaport Community Health Center. Since 1984, we have offered a team approach to medical care for the whole family. We are especially honored to care for patients at the beginning and end of life, and to care for multiple generations. Our success depends upon the trust and friendship we build into our relationships. We look forward to meeting and working with you.
A hidden epidemic is sweeping our young people, and we in Belfast are not immune. Two in five Maine students think that using substances is cool. Over a quarter of Maine high school students report consuming alcohol in the past month. We health care providers at Seaport Community Health Center, have been treating people with addiction problems since 2004. We are currently caring for over 70 people from our area who are addicted, but sadly, our program is now full and no longer accepting new people. The need for more treatment programs in Belfast and in Maine is serious and compelling. Even more compelling is the need for prevention. Read More >
Dear Seaport patients:
We will soon be asking if you’d like to included in a state-wide database called the Maine HealthInfoNet. It is a medical record available ONLY to clinicians who are directly involved in your care. The information would be drawn from our electronic health record (EHR) and transmitted over a private network to an encrypted database. The purpose is to provide your doctor with the most current clinical information, minimize prescription errors, and avoid duplication services. The doctors at Seaport Family Practice strongly support the integration of medical data in this fashion, and encourage your inclusion in the state-wide medical record. Here are a few more details:
1. HealthInfoNet is a Maine-based, independent, non-profit organization that supports the state-wide medical record
2. This record can only be accessed by participating clinicians who are directly involved in your care. Access records are continuously audited.
3. The state-wide medical record includes demographic data (age, gender, address, contact information), medications, allergies, problem list, lab tests, imaging reports, immunizations, and doctor visits.
4. The record does NOT include HIV test results, mental health or substance abuse diagnoses, or reports from mental health providers.
5. Your consent is required! You can opt-out; 1% of patients do. You can opt back in at any time you wish to do so.
