On behalf of the entire staff, we would like to welcome you to Seaport Community Health Center. Since 1984, we have offered a team approach to medical care for the whole family. We are especially honored to care for patients at the beginning and end of life, and to care for multiple generations. Our success depends upon the trust and friendship we build into our relationships. We look forward to meeting and working with you.
We offer Open Scheduling, so that most of our appointments are available at the start of each day. If all appointments are full by the time you call, we will try to answer your questions over the telephone, direct you to the emergency department, or “work you in” with one of the day’s providers- whichever best suits the need. We try to schedule regular appointments on the day the patient calls. However, if you need to schedule an appointment in advance, we will take your individual needs into account.
You can request an office appointment or annual physical exam online through new Athena Communicator Portal CLICK HERE. A physician is always available by phone (338-6900) After-Hours and on weekends; often we can arrange to meet you at the office.
We accept: Medicare, Medicaid, Blue Cross/Blue Shield & most managed care plans. We also offer a Sliding Scale fee schedule to qualified applicants.
Thank you for being a part of Seaport Community Health Center.
